Specialist home care services are available for a variety of circumstances, ranging from general personal care to clinical care requiring more complex needs, such as palliative care.
Temporary care may be needed after hospitalisation, illness, or a fall to help regain independence. This short-term care is also known as intermediate or aftercare, and includes reablement which assists with relearning daily activities such as cooking and washing.
Older people and adults with complex health needs who have a very urgent care need, including a risk of being hospitalised, will be able to access a response from a team of skilled professionals within two hours, to provide the care they need.
